氏名変更申請フォーム

受付可能な本人確認書類は以下のとおりです。有効期限内の書類を提出してください（旧姓の書類は有効期限が切れたものでも可）。
The following IDs are accepted. Please upload a valid document. (Expired documents are accepted only to prove your former name.)
□マイナンバーカード（個人番号カード）　Individual number card
□運転免許証　Driver’s license issued in Japan
□パスポート　Passport
□在留カード　Residence card
□特別永住者証明書　Special permanent resident certificate
□戸籍抄本/謄本　Abstract of family register / Copy of family register
□住民票　Certificate of residence
改姓・改名された方は、新氏名と旧氏名の両方がわかる本人確認書類（別々のものでも可）を提出してください。
If you have changed your first and/or last name, please upload the image data of your IDs showing your former and current names. These IDs do not need to be the same type.
マイナンバーカードの裏面（個人番号記載面）は提出しないでください。
Please avoid uploading the reverse side of Individual number card as it shows your individual information.
本人確認書類に本籍地の記載がある場合は、本籍地が見えないようにしてアップロードしてください。
戸籍上の氏名以外に変更することはできません。
ご登録者の変更はできません。
受験後に婚姻等による氏名変更がございましても、公式認定証の氏名は変更できません。
申請内容の確認後、不備等がなければ1週間程度で変更いたします。
